2. A copy of your signed Form 93 will automatically be emailed to you and Fresh Start™.
3. A Fresh Start™ team member will contact you with information regarding:
- Test Menu/Price List
- Procedure for Ordering
- Test Kits (no charge) | Direct Patient/Client Dropship Orders
- Test Interpretation Information, etc.
