DROPSHIP REQUEST STEPS
To have Fresh Start™ dropship a Test Kit directly to your Client/Patient, please follow the steps below.
1. Complete and sign the online Dropship Request Form
2. A copy of your signed Dropship Request Form will automatically be emailed to you and Fresh Start™
3. Fresh Start™ will invoice you for the dropship fee and requested tests
4. Upon receipt of payment, Fresh Start™ will complete the dropship service
