Fresh Start’s Dropship Service for Practitioners is for Certified Daphne Points Only
If you are not a certified Daphne Point™ (registered Practitioner), please register with Fresh Start™ here.
DROPSHIP REQUEST STEPS
To have Fresh Start™ dropship a Test Kit directly to your Client/Patient, please follow the steps below.
1. Complete and sign the online Dropship Request Form
2. You will receive an email from Adobe Sign
3. Click on the link in the email to confirm your signature/email address
4. A copy of your signed Dropship Request Form will automatically be emailed to you and Fresh Start™
5. Fresh Start™ will invoice you for the dropship fee and requested tests
6. Upon receipt of payment, Fresh Start™ will complete the dropship service
